Ohio Bureau of Workers’ Compensation 15K Program

Have questions about the Ohio BWC’s 15K Program? Here are a few facts:

• If a company opts in to the program, they pay up to $15,000 in medical and pharmacy bills for an injury.
• Once a company enrolls, they are responsible for the bills in all medical-only claims (claims with seven or fewer lost days from work) with injury dates after the enrollment.
• Injuries requiring more than 7 days of lost work are not eligible for this program.
• A First Report of Injury Form (FROI) (pdf) must be completed for any injury, including those in the 15K Program.
• Providers can lose BWC Certification if they don’t have the FROI completed for an injury.
